Clinical Manager – Home Health – Chantilly Office

100% office-based position (no exceptions)

• Accountable for branch clinical outcomes and customer satisfaction
• Ensures survey readiness and successful VDH, Joint Commission, and payer audits
• Oversees infection prevention and control compliance
• Monitors clinician productivity, utilization, and case mix
• Reviews hospitalization trends and implements corrective action plans
• Ensures timely completion of OASIS assessments and documentation
• Maintains appropriate staffing levels and clinician productivity
• Oversees branch on-call operations and clinical support
• Drives staff engagement, retention, development, and performance
• Participates in budget planning and expense management
• Ensures compliance with company policies, HIPAA, OSHA, and regulatory requirements

• Active Virginia RN license in good standing
• Minimum 2 years of home health leadership experience
• Minimum 2 years as a field RN in home health (no exceptions)
• Strong knowledge of OASIS, PDGM, Medicare reimbursement, and quality measures
• Experience managing interdisciplinary clinical teams
• Strong leadership, coaching, conflict-resolution, and communication skills
• Homecare Homebase (HCHB) experience preferred

• Medicare Star Ratings: Outcomes and HHCAHPS
• Potentially Preventable Hospitalization Rate
